- Quote request form: full name, company name, phone number, email address (optional), USDOT number (or that you do not have one yet), the lines of coverage you are interested in, and the type of risks you insure.
- Claim form: full name, company name, phone number, email address, policy number, type of claim, date of loss, truck and trailer involved, driver name, and loss location.
- Quote request by email on our articles: your email address.
We do not use cookies, analytics tools, advertising pixels or any other tracking technology on this website. Our web server keeps standard technical logs, such as IP address, browser type and the page requested, for security and troubleshooting, and rotates them automatically.
How we use your information
- to respond to your request and prepare an insurance quote;
- to open, report and follow up on an insurance claim;
- to share the details needed for a quote or a claim with the insurance carriers we work with;
- to keep a record of our communication with you.
We do not sell your personal information, and we do not use it for advertising.
Who receives your information
- Insurance carriers that we contact to obtain a quote or to report a claim on your behalf.
- Service providers that operate our website and email on our behalf: our hosting provider (DigitalOcean, servers in the United States), our email delivery provider (Brevo) and Google Workspace, which hosts our business email. They process the information only to provide their service to us.
- Authorities, when the law requires it.
How long we keep it
We keep form submissions for as long as we need them to handle your request and for the period required by the record-keeping rules that apply to insurance agencies. After that, we delete them.
Your choices and rights
You can ask us at any time to access, correct or delete the personal information you sent us. Write to insurance@polarisinsgroup.com or call +1 (331) 808-5896. Depending on where you live, you may have additional rights under state privacy laws, and we will honor them.
Security
Our website uses encrypted connections (HTTPS). Form submissions are stored on a protected server and are accessible only to authorized staff.
Children
This website is intended for businesses and is not directed to children under 13. We do not knowingly collect information from children.

If we change this policy, we will update the date at the top of this page.
